Unexpected Perils: Synthetic Opioids Shake Up the Drug Landscape
Opioid synthetics are altering the drug landscape.
Germany might steer clear of a full-blown opioid crisis akin to the one in the USA, but that doesn't mean synthetic substances like fentanyl aren't a looming menace. As evidenced at an international conference in Frankfurt, these substances pose a major threat, leaving authorities alarmed. According to Oliver Müller-Maar, the acting head of Frankfurt's drug referral office, "the dynamics are enormous."
A Rising Tide of Ketamine Consumption
Since 2009, 88 new opioid variants have surfaced on the European Monitoring Centre for Drugs and Drug Addiction's radar. These substances are whipped up swiftly and cost-effectively in labs, as Forensic Pathologist Volker Auwärter clarifies: "A slight chemical tweak, and you've got a brand-new substance." They come in various forms, such as powders, tablets, injections, patches, sprays, and liquid for electronic vaporizers.
Although advantageous for pain patients and anesthesia, these synthetic opioids become an unpredictable threat in uncontrolled consumption. Consumers often remain oblivious to what they're ingesting and how it affects them. Fentanyl, for instance, is 100 times stronger than morphine, while nitazenes might be a staggering 500 times so, explains Auwärter: "One misstep with a knife tip can be lethal, one microscopic amount harmful enough."
A Surge in Fentanyl-Related Cases
An overdose can lead to respiratory failure. Most fatalities – Auwärter brings up dozens in Germany, although a vast undercount – occur not in the classic drug scene, like Frankfurt's Bahnhofsviertel. Rather, these are experimenters who order these substances online, unaware of the potentially lethal dose.
Nina Kim Bekier, the chief physician of Frankfurt's clinic for addiction diseases at Bürgerhospital, also reports a growing number of emergency cases involving fentanyl and escalating demand for opioid detoxification.
The reasoning behind synthetic opioids' growing prevalence in the German scene lies in Afghanistan, as Health Commissioner Elke Voitl (Greens) explains: "As opium poppy cultivation decreases there, heroin is scarce, leaving a gap filled by synthetic opioids."
An Agonizing Child's Death and Parental Prosecution
In January, fentanyl was detected in 21 heroin samples tested in a Frankfurt consumption room. Around half of the samples tested positive, according to the Frankfurt police's prevention officer, Lars Küthe. The police headquarters informed the state criminal police office (LKA).
At the conference, all agreed that drug testing—or drug checking—would be essential for gaining a foothold on the situation. Such testing would provide consumers the chance to chemically analyze their drugs, saving them from unknowingly consuming unknown substances with unpredictable consequences. The results might also aid tracking the market's developments.
Although the federal government initiated the framework for this two years ago, the current black-red coalition in Hesse has yet to issue implementing regulations. This leaves the Frankfurt pilot project on hold.
The conference saw a heated debate over how these tests should be carried out. While Auwärter cautioned against rapid tests – deemed too prone to errors – representatives from drug assistance facilities emphasized that their clientele is unwilling to wait days for laboratory results.
